Our structured survey supports client to self-assess how advanced health and care systems are in adopting evidenced-based care models, putting enablers in place and achieving impact. Systems scoring better on the maturity matrix have demonstrated improvements in performance (e.g. reduction in non-elective admissions among older people).

How the planning service works The Integrated Care Delivery Toolkit
The availability of tools to measure progress in integrated care delivery – both in implementation and in impact – is currently poor. We have developed an entirely scalable, evidence based way of supporting the implementation of new care models. Our toolkit enables areas to (a) segment their local population into groups with similar health and care needs; (b) rapidly assess the maturity of interventions for specific cohorts; (c) measure impact (in terms of activity and money) and associate impact with maturity; (d) embed multi-disciplinary team working and the leadership required to accelerate change. This includes leadership of the clinical interventions themselves and the team-based processes by which changes occurs. These tools are designed to work at the level of any unit built up from GP practices. We deliver a robust set of outputs and offer to calibrate results, analyse drivers of difference and facilitate discussion of the implications.

The Integrated Care Delivery Maturity Matrix
The maturity matrix addresses part (b) of our toolkit (outlined above). We help buyers plan how they will implement the tool through one of three options:

Support levels Three types of support are available:

Clients receive a detailed Welcome Pack which outlines the implementation process for the maturity matrix tool. An initial mobilisation discussion is then set up with the client to agree the objectives, scope and a detailed and customised implementation project plan.

We provide access to our portal for the cloud based tool.We provide dedicated support from a project team, led by a Lead Engagement Partner who is accountable for the delivery of our service. This support is part of our service, and is provided throughout the duration of our service provision.

The tool itself is simple to use and has an online guide. For questions relating to the functionality of the survey and how to complete it, our third party supplier offers email support (24/7) and phone support (9am – midnight Mon-Fri).
